Medical Records Specialist
Job Description:
A growing healthcare organization is seeking a detail-oriented Medical Records Specialist to support clinical documentation, patient file management, physician order tracking, and general administrative workflows. This role is ideal for someone who is organized, dependable, comfortable working with confidential records, and able to support a busy home health or hospice care environment.
The Medical Records Specialist will help ensure patient records are accurate, complete, timely, and compliant with healthcare documentation standards.
Key Responsibilities
- Organize, sort, scan, upload, and maintain patient records and clinical documentation
- Ensure patient files are complete, accurate, and updated in a timely manner
- Process physician orders, face-to-face documentation, consents, DNR forms, and other required medical documents
- Send documentation to physicians by fax, mail, or other approved methods
- Track outstanding orders and follow up with physician offices to obtain signatures
- Scan signed orders and completed documentation into patient charts
- Provide clerical support, including filing, copying, typing, faxing, scanning, and document preparation
- Prepare admission packets, orientation materials, new-hire packets, and other administrative documents as needed
- Confirm required admission paperwork and consents are properly completed
- Support communication related to missed visits, drug interactions, hospitalization follow-ups, and discharge follow-up calls as assigned
- Assist scheduling, intake, clinical leadership, and branch leadership with administrative tasks
- Maintain confidentiality and follow HIPAA, privacy, and company compliance standards
- Communicate professionally with patients, families, physicians, vendors, and internal team members
- Support timely documentation and help maintain smooth office operations
- Perform other administrative and medical records duties as assigned
Required Qualifications
- High school diploma or GED required
- Active HIPAA compliance / training credential required
- Strong computer skills and comfort with electronic records, scanning, faxing, and data entry
- Excellent organizational, communication, and interpersonal skills
- Strong attention to detail and accuracy
- Ability to manage multiple deadlines and prioritize work effectively
- Reliable transportation and valid/current auto insurance, if travel is required
- Ability to work under time-sensitive conditions and meet documentation deadlines
- Professional, dependable, and able to maintain strict confidentiality
Preferred Qualifications
- Previous experience in medical records, healthcare administration, home health, hospice, clinical documentation, or medical office support
- Experience working with electronic medical records or healthcare documentation systems
- Familiarity with physician orders, patient charts, consents, DNR forms, and face-to-face documentation
- Experience communicating with physician offices, patients, families, and clinical staff
- Knowledge of HIPAA, healthcare privacy, and documentation compliance requirements
Physical Requirements
- Ability to sit for extended periods while working on records, documentation, and computer tasks
- Ability to stand, bend, stretch, and reach for files or office supplies as needed
- Ability to occasionally lift, carry, push, or pull office materials, files, or equipment
- May require occasional travel to customer, provider, or office locations, depending on business needs
